Any request for a designation of real property as an economic revitalization area shall include the following:

    A.    The titled landowner of the property involved;

    B.    A legal description of the titled property involved and the commonly known address of the real property;

    C.    A statement that all taxes are current and paid with regard to said titled property;

    D.    A complete statement of benefits form;

    E.    An executed agreement that the applicant will participate and cooperate with the city and/or its designated agencies and the common council for purposes of an annual review required by Indiana Code, 6-1.1—12.1-5.1 (real property deduction), or Indiana Code, 6-1.1—12.1-5.6(b) (new manufacturing equipment deduction);

    F.    If the business organization is publicly held, the name of the corporate parent and the name under which the corporation is filed with the Securities Exchange Commission;

    G.    A map and/or plat describing the area where the economic revitalization area designation is being requested;

    H.    The current assessed valuation of the real property before rehabilitation; redevelopment, economic revitalization or improvement;

    I.    The real and personal property taxes paid at the location during the previous five years, whether paid by the current owner or a previous owner;

    J.    A description of the proposed project (whether rehabilitation, new construction or installation of new manufacturing equipment), including information about physical improvements to be made or the new manufacturing equipment to be installed, an estimate of the cost of the project, the amount of land to be used, the proposed use of the improvements, and a general statement as to the value of the project to the business;

    K.    An estimate of the number of full- and part-time permanent jobs to be created by the project within one year, a statement of the current number of permanent jobs at the location and the impact on those (current) jobs to be caused by the project, the projected annual salaries for such positions to be created;

    L.    A statement that no building permit has been issued for construction on the real property for the improvement proposed; and

    M.    The name, address and telephone number of the person or contact regarding notice of council meetings and meetings concerning the petition. (Ord. 92-7 § 3, 1992)
