The petitioner shall file with the department of community development its annual report on forms previously provided to it by the department of community development. Such mailing by the department shall be sent annually to each approved applicant on or before February 1st by certified mail. Each applicant must return the completed annual report to the department of community development within forty-five days. Such information shall be submitted by a corporate officer with said applicant, under oath, and directed to the department of community development. (Ord. 92-7 § 5, 1992)
